CLAIM SUBMISSION
To start a claim, the purchaser must go to PRODUCT & SHIPPING WARRANTY CLAIM FORM
You will need the following information to complete a claim:
- Certificate Serial Number
- Proof of purchase (Name of Purchaser, Order Number, Date of Order)
- Photos of damage
- Description of incident
